Membership
Use this form to apply for new membership with Fariji DT Sacco. Complete all sections in full and attach a copy of your National ID and a passport-size photo, then bring it to any branch with the registration fee.
Loan Forms
Use this form when applying for a Development Loan, School Fees Loan, Emergency Loan, or Salary Advance. Complete all sections in full, attach your National ID copy and two recent payslips or bank statements.
Membership
Use this form to transfer your accumulated shares to another eligible Fariji DT Sacco member. Both the transferor and the recipient must sign the form. The receiving member must already be an active member in good standing.
Procurement
Procurement document for vendors and service providers wishing to bid on Fariji DT Sacco tenders. Review the scope of work and submission requirements before preparing your proposal.

Fill in all required fields clearly using a black or blue pen. Do not leave any mandatory section blank.
Clip your National ID copy and any supporting documents (payslips, bank statements, cooperative records) to the form.
Bring the completed form to our office at Diplomat House, Githunguri, during working hours.
